Please note that the online impact report form does not allow you to save your work. We recommend that you complete your responses in a Word document and then copy and paste them into the online form.

Required Attachments

All attachments must be emailed to with the subject line: ArtsMatch Impact Report Attachments-[Your Organization Name]

  1. Photos from the project. Please include a caption for each photo submitted. Please note that all images must be owned by your organization. Any penalties incurred for use of unauthorized images will be the responsibility of the grantee.
  2. Examples of how your project recognized the matching sponsor recognition (i.e., copies of newsletters, screenshots of posts, etc.).
  3. Project Budget. Please include an itemized budget of all income and expenses associated with the project.
